A standard hazard with anything that thumps along I reckon. Rubber washers/bushes and lock nuts are the norm.

Maybe it just died of loneliness. Maybe it needed a mate on the front of the bike!!!!!
How bout getting some personalised ones "BIG" on the back of your bike would look cool.
